Privacy Policy
Angkor JAPAN Friendship International Hospital (hereinafter referred to as “the Hospital”) establishes the following privacy policy (hereinafter referred to as “this Policy”) regarding the handling of personal information of its users (hereinafter referred to as “Users”).
- Article 1 (Personal Information)
- Personal information” refers to the information defined by the Personal Information Protection Law. It includes information related to a living individual that can identify a specific person through descriptions such as their name, date of birth, address, telephone number, contact information, and other details. It also encompasses data related to appearance, fingerprints, voiceprints, and health insurance numbers that, when used alone, can identify a specific individual (personal identification information).
- Article 2 (Method of Collecting Personal Information)
- Our hospital may ask for personal information such as name, date of birth, address, phone number, and email address when users make inquiries or apply for job opportunities. Additionally, we may collect information related to user transactions, including personal information, and payment details through our partners (such as information providers, advertisers, and ad distributors).
- Article 3 (Purpose of Collecting and Using Personal Information)
The purposes for which our hospital collects and uses personal information are as follows:
- 1.To operate and manage the hospital’s services.
- 2.To respond to inquiries from users (including verifying their identity).
- 3.To communicate important information and updates as necessary.
- 4.For purposes related to the above, such as improving service quality and user experience.
- Article 4 (Changes to the Purpose of Use)
- Our hospital may change the purpose of use of personal information, only if it is reasonably recognized that the new purpose is related to the original purpose before the change. If a change is made to the purpose of use, the updated purpose will be notified to the users through a method prescribed by the hospital or will be publicly announced on this website.
- Article 5 (Provision of Personal Information to Third Parties)
Our hospital will not provide personal information to third parties without prior consent from the user, except in the following cases. However, this excludes cases permitted under the Personal Information Protection Act and other laws and regulations:
- 1.When it is necessary for the protection of a person’s life, body, or property, and it is difficult to obtain the user’s consent.
- 2.When it is particularly necessary for improving public health or promoting the sound growth of children, and it is difficult to obtain the user’s consent.
- 3.When it is necessary to cooperate with a national institution, a local public body, or an entity entrusted by them in executing affairs prescribed by law, and obtaining the user’s consent may impede the execution of those affairs.
- 4.When the following matters have been notified or publicly announced in advance, and the hospital has notified the Personal Information Protection Commission:
- 1.Inclusion of provision to third parties in the purpose of use.
- 2.The data items to be provided to third parties.
- 3.Means or methods of providing to third parties.
- 4.Procedures for users to request a stop to the provision of personal information to third parties.
Notwithstanding the provisions of the preceding paragraph, in the following cases, the recipient of the information is not considered a third party:
- 1.When our hospital entrusts the handling of personal information, in whole or in part, within the scope necessary for achieving the purpose of use.
- 2.When personal information is provided in connection with business succession due to merger or other reasons.
- 3.When personal information is used jointly with a specific person, and the user is notified in advance or the user can easily know the purpose of joint use, the scope of the jointly used information, the identity of those using it jointly, and the person responsible for management of the personal information.
- Article 6 (Disclosure of Personal Information)
When requested by the user to disclose personal information, our hospital will promptly disclose the information to the user. However, if disclosure falls under any of the following, the hospital may not disclose all or part of the information. In the case of a decision not to disclose, we will notify the user without delay. Please note that a fee may be charged for the disclosure of personal information:
- 1.When there is a risk of harm to the life, body, property, or other rights and interests of the user or a third party.
- 2.When disclosure is likely to significantly interfere with the proper execution of our hospital’s operations.
- 3.When disclosure would violate other laws and regulations.
Notwithstanding the provisions of the preceding paragraph, information other than personal information, such as historical data and characteristic information, will, in principle, not be disclosed.
- Article 7 (Correction and Deletion of Personal Information)
- Users may request correction, addition, or deletion (hereinafter referred to as “Correction, etc.”) of their personal information held by our hospital if they find that the information is incorrect. This request should follow the procedures prescribed by our hospital. Upon receiving such a request from the user and determining that it is necessary, we will promptly make the required corrections, etc. Additionally, if a correction is made or if a decision is made not to make a correction, we will promptly notify the user of the result.
- Article 8 (Suspension of Use of Personal Information, etc.)
- When a user requests the suspension or deletion (hereinafter referred to as “Suspension, etc.”) of their personal information due to reasons such as it being handled beyond the scope of its intended use or because it was obtained through improper means, the hospital will promptly conduct the necessary investigations.
If it is determined based on the investigation results that the request should be granted, the hospital will promptly carry out the Suspension, etc., of the relevant personal information.
In cases where the Suspension, etc., is carried out, or if a decision is made not to proceed with the Suspension, etc., the hospital will promptly notify the user of the outcome.
Notwithstanding the preceding paragraphs, if the Suspension, etc., requires significant costs or is otherwise difficult to execute, and if an alternative solution is available to protect the user’s rights and interests, the hospital will take this alternative action.
- Article 9 (Changes to the Privacy Policy)
- The content of this policy may be changed without prior notice to the user, except as otherwise provided by law or in this policy. Unless otherwise specified by the hospital, the revised privacy policy will become effective upon being posted on this website.
